## Description Vanderbilt Home Care is seeking a Home Care Intake Coordinator.This is a customer service position, as all home care referrals come through this department. This individual will be working in a fast paced environment and constantly multi-tasking. The individual will be working with a team of 4-5 Home Care Intake Coordinators managing the referral process and flow. They will be receiving important patient information from the referral source and assessing their eligibility for home care and working with the case managers/discharge planners (referral sources) to assist in the transition to home care. The successful candidate will be responsible for processing and facilitating home care referrals from both VUMC and outside of our system, including assuring that referral sources have access to information and assistance when referring a patient to home care. All patient information is obtained and referral is accepted, with follow-up when necessary. Collaborates with external / internal health care teams to meet patient care needs is critical. **Key Functions and Expected Performances:** 1. **Planning & Managing Care** 1. Receives referrals for home care services and consults (as appropriate/able) with the referral source, physician, primary care providers in that health care setting, case manager, social worker, and patient/family regarding home care needs 2. Makes decisions re: acceptance of referral for evaluation by VHCS 3. Oversees required documentation including: referral form, logs/data base, and others as needed 4. Supervises the intake process assuring completion of required documentation 5. Organizes records of community resources 6. Organizes and oversees detailed referral/customer base reports 2. **Continuum of Care/Provision of Care** 1. Facilitates coverage determination in conjunction with assistant managers, Director of Patient Care Services, Administrator, personnel and office support 2. Attends hospital/clinic (external) patient care conferences/meetings as requested 3. Actively exchanges pertinent information with external/internal health care teams, patients, and families/caregivers 4. Uses information obtained in the collaboration process to plan for home care 5. Coordinates needed information with customers through multiple distribution modalities (i.e. mailing, fax, telephone etc.) 3. **Patient/Caregiver, Team & Community Education** 1. Communicates with agency staff re: start of care needs and other specific patient issues 2. Identifies pertinent information needed by internal/external health care teams to move patients/families/caregivers toward desired goals 4. **Problem Solving 1. Identifies problems and/or actual or potential variances from goals and communicates with the health care team 2. Initiates finding solutions for problems/variances and implements actions that move patients/families toward desired goals 3. Initiates collaboration with the health care team to assure timely identification of the root cause(s) of problems/variances. 4. Consults with supervisor and/or admitting physician/designee when serious difficulties arise preventing admission to HHC to review alternatives to admission and document in the medical record 5. **Communication and Collaboration 1. Communicates with agency staff regarding start of care needs and specific patient issues related to the plan of care 2. Develops appropriate channels of communication with customers 3. Communicates with referral source any pre-certification issues that would impede requested HHC orders. 4. Follows up on all hospital readmissions and interacts with medical and professional staff, on a case-by-case basis, to obtain additional clinical information to support appropriate home care admission and or resumption of care and documentation in the clinical record. 5. Serves as a resource to physicians and office staff and interacts with patient and/or family regarding home care admission criteria. 6. **Continuous Learning** 1. Participates in continuing education; obtaining the minimum number of hours per year as required per discipline 2. Contributes to CQI activities 3. Identifies need for professional growth and seeks appropriate professional development opportunities 4. Sets annual goals and strives to achieve them 7. **Team/Organization 1. Functions as an integral member of team 2. Complies with policies and procedures 3. Orients new staff 4. Adheres to and promotes VHCS Mission and Vision, attends/participates in team, agency and committee meetings, including agency and discipline-specific Unit Boards utilizing the Shared Governance Model 5. Contributes to clinical practice area, work life and team building 6. Integrates professional practice/job responsibilities and organizational expectations 7.
